Oral Nicotinamide – A Vitamin to Improve Skin Health

What is it?

Nicotinamide is one of the most common forms of Niacin, used in dietary supplements. It has been increasingly studied for many different indications in the field of dermatology, including prevention of skin cancer and improvement in the skin’s appearance.

Nicotinamide is naturally found in lean meats, fish, nuts and legumes. Oral nicotinamide is available as 20–30 mg in multivitamins, and on its own as 500-mg tablets. It has also been incorporated in many topical agents including sunscreens and cosmetics.

A Vitamin To Improve Skin Health

What are the benefits?

  • Reduces inflammation in the skin
  • May reduce risk of skin cancer
  • Improves skin hydration

What conditions does Nicotinamide treat?

  • Acne: Nicotinamide has anti-inflammatory action and reduces oil production. It has been shown to be effective in clearing acne lesions.
  • Dry skin: It can improve skin hydration by decreasing water loss through the top layer of skin (epidermis). One study showed nicotinamide to increase the skin’s production of ceramides, thus improving skin hydration.
  • Rosacea: It is reported to improve complexion, by decreasing red pigmentation. It also may improve skin barrier function, leading to diminished reaction to irritants including cleansers and cosmetic products (common in rosacea patients).
  • Pre-cancer and cancer: Some studies have shown that patients taking oral nicotinamide 500mg twice daily who have extensive precancerous lesions and cutaneous squamous cell carcinomas may expect a reduction in the number and severity of new lesions over time. Additional long term studies are needed.
  • Anti-Aging: Some studies show that nicotinamide cream improves fine lines and wrinkles, hyperpigmentation, and skin elasticity.

Side effects?

Oral nicotinamide is generally well tolerated in doses under 3 Grams per day.  At high doses, potential side effects include:

  • Headache
  • Nausea, vomiting
  • Fatigue, dizziness
  • Liver toxicity
  • Increased risk for low platelets

Do NOT take if:

  • You are taking anticonvulsants such as carbamazepine: Nicotinamide may increase the blood levels and risk of side effects of this drug.
  • You have low platelets: A meta-analysis suggests that using nicotinamide may increase the risk for low platelets, so patients should consult with their healthcare provider.

Take away:

Although oral nicotinamide appears to reduce skin cancers caused by ultraviolet (UV) light exposure, it is not a substitute for sunscreen and does NOT protect against sunburn. However, taking this supplement may be beneficial to your skin health.
